❂ Self-Schedule: Once enrolled, you'll gain access to our online clinic calendar to schedule your first visit. Initial appointments are available in-person at our Tysons clinic, and follow-up visits can be scheduled easily through your VIP patient portal.

Onboarding

❂ Complete Forms: After enrollment, you'll receive electronic intake forms. Please complete and submit them within 7 days to ensure we can move forward with your care.
❂ Activate Patient Portal: After enrollment, you'll also receive a link to activate your VIP patient portal. Your VIP patient portal lets you manage appointments, securely communicate with your provider, and share important health information ahead of your visits.

How do I dis-enroll from my VIP Treatment Package?

You have the flexibility to dis-enroll from your VIP Treatment Package after the initial 4 months by following these simple steps:

❂ Submit Dis-enrollment Request: Send us a written dis-enrollment request through your VIP patient portal.

❂ Credit/Debit Card Billing Adjustment: Once your dis-enrollment request is processed, we will stop your monthly billing during the next monthly billing cycle, which will occur 30 days after receiving your initial request.

❂ Pro-rated Refund for Pre-paid Plans: If you opted for the pre-paid option, you will receive a pro-rated refund based on the remaining term of your VIP Package after the 30-day period.

These services are typically not covered by most health insurance plans.
